In order to provide our senior living placement and advisory services and help us
identify appropriate care and housing options, we may collect information including:

  • Date of birth
  • Emergency contact information
  • Health and medical information relevant to senior living placement
  • Mobility, cognitive, and care needs
  • Insurance or payment information necessary for placement discussions
  • Information provided by family members, caregivers, physicians, hospitals, or
    other healthcare providers with your authorization

We make reasonable efforts to maintain the information we collect in an
accurate, complete, and current form. Requests for updates or corrections to

How We Use Your Information

We collect and use your information in a lawful and fair manner, with your knowledge
and consent, solely to:

We obtain information verbally from you and obtain your consent either verbally or in
writing. We keep your verbal or written consent in electronic or printed format.

Trua is committed to maintaining the confidentiality of any Protected Health Information

(PHI) or other health-related information you provide.

We will not share your PHI or other health information with anyone who is not directly
involved in evaluating, coordinating, or providing the services you have requested,
unless:

  • You or your legal representative authorize us to do so;
  • Disclosure is required by law, court order, or government regulation; or
  • Disclosure is necessary to protect the health or safety of you or others as
    permitted by applicable law.

    Our employees and franchise owners are expected to maintain strict confidentiality and
    only access client information necessary to perform their duties. We retain sensitive
    information, including health and financial information, only for as long as reasonably
    necessary to provide our services and meet business requirements.

    Who We May Share Information With

    With your permission, we may share relevant information with:

  • Senior living communities
  • Assisted living facilities
  • Memory care communities
  • Independent living communities
  • Skilled nursing facilities
  • Home care agencies
  • Hospice providers
  • Physicians
  • Hospitals
  • Rehabilitation facilities
  • Family members or legal representatives you authorize
  • Other providers directly involved in your care or placement

We do not sell, rent, or share your personal or health information for marketing
purposes.

We will provide a disclosure and advisory form and obtain a signed acknowledgment
when meeting in person. When appropriate, we may also discuss the matter by phone
before the form is presented for signature. The form will be stored in electronic or
printed format. Periodically, after the mandatory 3 year period, the forms will be purged
and disposed of responsibly.

Information Security

We maintain administrative, technical, and physical safeguards designed to
protect personal information from unauthorized access, disclosure, alteration, or
destruction. Printed documents are stored in a locked building accessible only
by trusted persons. Electronic documents and information are stored with
password protection. While no method of electronic transmission or storage is
completely secure, we take reasonable steps to safeguard the information
entrusted to us.
